Honestly, I personally wouldn't rely on being able to reduce the length of blocks in order to do a job. I suggest, with the medical world being as big as it is, choosing a job where speaking quickly under pressure is not a requirement. It's like if I were to go into investment banking with a chronic illness that means I need 13 hours of sleep a day! I would simply not pursue that career, and find another. The world is a big place.
