Can't start to speak because of blocks

Can't start to speak because of blocks I feel I can't physically talk anymore when outside. I'm afraid that when I try to say something my voice will get block and embarass myself. My fear is mostly at shops like I can't start to say what I want instead I'm left with an open mouth without words coming out.
