well thoughts and feelings are two different things. I am a rather severe stutterer so ive always acklowedged and accepted that I stutterered so there was no "fear" of whether i would block or not. I knew I would. But I've begun to understand that my fear and shame is extremely deep and im still trying to understand it. There have been certain moments on my life where I felt so good that fear was temporarily gone and stutters just didnt bother me. But most of the time I definitely do hold back and block (that's a form of avoidance). So I'm still weary when people say they dont have any fears anymore. That said, you might benefit from a more targeted therapy. You can try behavior modification to help you with blocks or if you reallllllly want to work on fluency and dont care how you get it or what you have to change, you can try fluency shaping therapies. Im not a fan of them because it still felt like i was hiding by changing how i spoke to be more fluent, but there are people who really like it and practice everyday and work the techniques into their normalized speech and it helps them. When you block, is your stomach tense? It probably is. I have this problem at least. learning to release the tension would probably help. Check this guy out: http://www.valsalva.org/ He's got a good book and has an interesting take. I've been to a few of his workshops and its always very interesting. Could be what youre looking for.
