Answering the phone, interrupting people, bringing up a new topic of conversation, or, strangely enough, talking to Siri or Google Now. Basically, if I have to start a dialogue it's difficult (but not nearly as severe as a lot of people) but if I'm responding to someone I'm more or less fine.
