I stutter when I'm talking on the phone

I stutter when I'm talking on the phone For some reason I tend to stutter when I'm talking on the phone with people, but not so much in face to face conversation. I have identified the words that I stutter on mostly are those beginning with a vowel or, if not a vowel, then a hard sound (like kick). Unfortunately a large part of my job is talking to people on the phone and I have now developed a very annoying habit of saying "um" before I say certain words that I stutter on to try and soften up the hard sound of the next word so I don't stutter on it, if you know what I mean. I had no idea on how much I did this until last week. I transcribe audio dictation and a colleague had dictated something while sitting beside me and I was on the phone and it was painful to listen to my own voice in the background! It was actually very embarrassing now that I know how much I am saying "um". Would anyone have any tips or guidance on how I can get out of this habit? I did stutter when I was a child and am now 37 so I'm sure certain speech habits are well embedded.
