Since its happening when you first start speaking, I think it has to do more with your breathing. Do you find yourself trying to say things without breathing first? Diaphragmatic airflow is crucial to speaking and can lead to blocks if you don't have enough breath behind your words. Next time you speak, pause for a half a second and take a full breath and start speaking on the exhale, you'll find it helps you.
