I would suggest going to therapy, If you want something more intense, there are immersive programs out there. There's AIS in New York and SSMP in Washington State is very good too. I can empathize with the feeling and when you reach a certain severity, it does become how to exaggerate the impact, especially if you are otherwise social, ambitious, etc. I would say do whatever you can to get in a better place with your speech. I dont know your financial situation, but find some way to get help, most likely in the form is good therapy (hard enough to find on it's own). If you have the time and the funds, come out to the NSA conference this year. If you've never been before, this would definitely be my #1 suggestion.
