I've always had it, so I never noticed it. I know my mum noticed it when I was young, but I'm not sure how young. Teachers never commented as far as I'm aware and I didn't notice any bullying about the stutter specifically (was too busy getting bullied for everything else about me). Environments can play a factor, but it's just generally where I'm not quite as comfortable (so hanging out with a bunch of close friends is fine, but hanging out with a bunch of close friends plus a friend of a friend that I'm not comfortable with isn't). It's always there to some extent, but it can fade to the point where it just sounds like I'm struggling to think of the correct word rather than unable to say the correct word. I did go when I was younger and it did apparently help a bit. Can't remember what I did there though. Don't drink or take any drugs stronger than ibuprofen so can't answer that question.
